Village Team Leader

Mission Category: Direct Service

Reports To: Director of Village ISA

Supervision Responsibilities: Provides general oversight or workleader responsibilities for lower level staff during program or service activities. Provides direct supervision to assigned team members based on Village model.

Education and Experience Matrix one of the following combinations is required::

EXPERIENCE

bachelor degree in human service or related field

Minimum 5 years experience providing psychosocial rehabilitative services to a disabled population.

bachelor degree plus license or certification in a mental health specialty (ie, LVN, RN, LPT, drug/alcohol rehab counselor)

Minimum 3 years experience providing psychosocial rehabilitative services to a disabled population.

master’s degree in Social Work (MSW)

License waivered or waiver-eligible plus a minimum 2 years experience providing psychosocial rehabilitation related services to a disabled population.

master’s degree in Social Work (MSW)

No psychosocial rehabilitation experience required. Must possess a current, valid LCSW

doctoral degree in psychology

No psychosocial rehabilitation experience required. Must at minimum be licensed waivered or waiver-eligible as a PhD psychologist

Other position requirements:

  • Demonstrated ability to provide supervision and leadership to diverse staff who possess a wide range skills, abilities and professional competencies.
  • Class C driver's license and proof of valid automobile insurance.
  • Knowledge of community resources.
  • Flexible work schedule to include some weekends and holidays.

Typical Tasks and Duties include the following. This is not an exhaustive task list, and other duties may be assigned as deemed appropriate or necessary by the supervisor or program director. Reasonable accommodation will be provided to any qualified person with a medical or psychiatric disability, providing it will not change the essential nature of the position, nor cause undue hardship to the Association’s operations.

  • Recruits, hires, supervises, trains, directs, evaluates, rewards, disciplines and terminates assigned staff in accordance with organizational policy and applicable laws and regulations.
  • Oversee and manages team budget and expenditures, making adequate and timely adjustments as needed.
  • Participates as a manager in team, staff and community meetings. Leads or delegates leadership of the day-to-day operations of the team as needed. Participates as an active member of Management Staff, quality improvement, safety committee or other leadership teams as needed or assigned. Provides timely information and feedback to Village Director with regard to emergent staff, team or member issues.
  • If licensed and as appropriate based on licensure status and scope of practice, provides individual and group psychotherapeutic services, writes 5150 holds as needed or requested, reviews, monitors and co-signs on member service plans developed by lower level staff, as necessary reviews, monitors and manages psychotherapeutic treatment programs of members, and performs other similar professional services and activities as needed within the scope of practice of the license or degree.
  • Develops a Coordination Plan and/or Personal Service Plan with each member that identifies his/her personal needs and goals.
  • Mediates and advocates on behalf of the member to help him/her obtain and keep housing, health care benefits, entitlements etc.
  • Visits members in their place of residence on an as-needed basis in order to help them live successfully, and to prevent them from becoming homeless.
  • Transports members to appointments and on errands when appropriate.
  • Oversees and monitors members’ individual and group activities both inside the program and out in the community.
  • Provides individual and group coaching, counseling mentoring instruction, training, feedback and positive reinforcement for members in areas such as money management, grooming and hygiene, meal preparation, public transportation, and other independent living skills.
  • Devises and implements strategies for outreach to members who tend to isolate themselves.
  • Oversees and monitors the collection of service coordination outcome data.
  • Maintains all records and documentation (progress notes) needed for program compliance and accountability in accordance with MHA standards and funding source requirements.
