Root: Assertive
Case Management
- Community-based teams provide a full range of medical, psychosocial
and rehabilitative services to prevent hospitalization of clients by
increasing their skills and their supports in the community.
- Services are provided on a long-term basis and there is continuity
of caregivers over time and across settings.
- Seventy-five percent or more of the services are
delivered outside of the program offices.
- Emphasis is on outreach, relationship building and individualization
of services.
- Multidisciplinary teams that include a staff psychiatrist deliver
case management services.
- Case managers have small caseloads and have primary responsibility
for each client’s treatment plan, individual supportive therapy and
crisis needs.
